In other words:
For each Care Plan there can be one or more Nursing Diagnosis.
For each  Nursing Diagnose there can be one or more  Related Factors, Symptoms/Risk factors, Outcome /Goal  and  Interventions. For each Intervention  there can be one or more Task's.

Nursing Diagnosis / Medical Diagnosis ---- the difference:

It is important to distinguish nursing diagnoses from medical diagnoses. The two are similar because they are both designed to plan care for a patient. However, nursing diagnoses focus on human response to stimuli (symptoms), while medical diagnoses focus on the disease process. An example of this difference is the different diagnoses given by a nurse and a doctor to a patient who has Appendicitis which would be the medical diagnosis. Some nursing diagnoses that might be made for this particular patient, however, include Acute pain, Nausea, Impaired physical mobility and fatigue.

Nursing Diagnosis are in general made from Symptom/s (human response to stimuli). Symptom is the key component in Nursing Diagnosis.

Symptom/s is the most important key factor regarding Nursing and Nursing Care Plans.
                                 

                             The standard nursing care plans in CareScribble:

The nursing care plans provided here with CareScribble are ´general´ or ´standard´for many problems.
The care plans are provided here  ´as is´ and  could and should only be used as guidelines when composing care plan for patient. Most of these standard nursing care plans are about 'one page long' (printout) but some of them are longer , up to 3 pages long. Care plans used for your patient at time should be short as possible. In general Care Plans which extends more than one page ( 4 to 5 diagnosis) are mostly useless because in general Nurses do not read them and follow.

It is very important that each care plan is tailored to a patient´s specific needs and have them short as possible. No patient fits exactly into ´standard´ care plan because there are some items (such as nursing diagnosis) which do not apply.
